Many students have
a hard time concentrating or staying alert in school
all day. You can do a self-check of your alertness
during the day by filling out an Alertness Log.
DIRECTIONS:
- To start, fill in the chart
with the days classes or activities.
- After each class or activity,
mark the box that best describes you.
- If you were alert or "On"
for the whole time
check On.
- If you were "On" for
some of the time, but were tuned out or distracted
at other times
check On & Off.
- If you couldnt concentrate
at all
check Off.
